Documentation of Evaluation Results form

Hello,

I have two questions/requests for the 'documentation of evaluation results' form:

1. The information on this page gets a bit muddled together from section to section when printed - the only way each area is defined in the printed format is by a bolded title for each section (Academic Achievement, Functional Performance, etc) - it is still the same font size as the rest of the page. It makes it difficult to quickly find the section you may want to read through. Is it possible to have those sections more clearly defined with a gray bar/centered text, such as it currently is with the 'Hearing/Vision' section on this same page, or in the sections of the 'documentation of intervention results' page? These gray bars/headers make it much more visually easy to navigate from section to section.

2. You have provided a fix for multiple users to access the same form on some other pages, such as the consent for evaluation/domain and the present levels of performance. The documentation of evaluation results page is another form also often accessed by multiple users. Could you make that change in this form as well?

  Thank you for your feedback!

  1. Currently, user entered text in an expanding text field will appear italicized when printing. The form fields, on the other hand, will appear bolded. We have tried to keep the form text and spacing at a size and layout to increase readability without being wasteful. Based on your feedback, we will ask our programmers to add additional section headers to increase readability on the Documentation of Evaluation Results.
  2. We are glad your district has found the multi-user form editing feature beneficial! Because the Documentation of Evaluation Results form can import data from other forms (Consent for Initial Evaluation or Consent for Reevaluation), we had to complete a few additional programming steps before we could apply the multi-user functionality to this form. This form is scheduled to receive the multi-user editing feature, and we are currently working with our programmers to apply the update to the next round of documents.

  I wanted to let you know we have updated the Documentation of Evaluation Results form to add section headers to increase readability. This will be effective on all new forms created from this point forward.
