Date field for the 3 Could Doc of Contacts on Conf Summary page

Could a date field be added to the left each of the 3 fields of the documentation of contacts part of the Conf Summary Report?

Sometimes, people forget dates, this might prompt them to be sure that they list a date for each contact.

  We appreciate the suggestion! We will add a form feature to hide or show dates for the contact attempt fields on the Conference Summary Form to our roadmap of future updates.
